I understand that it is my responsibility to keep my account secure by keeping my details confidential. I understand that I can terminate my account at any time by contacting the surgery, or change my log in details by re-registering and that this form will be kept on my electronic records. I understand that my registration will be revoked if I constantly miss or cancel appointments.

Proof of identity

To complete your registration, please upload:

  • photographic ID
  • proof of address

You will also need to bring your photographic ID to the surgery before we can process your request.

If you’re requesting access for a child under 11, you’ll also need to bring the child’s birth certificate to confirm parental responsibility.

If the child is aged 11 to 16, they will need to complete a consent to share form at the surgery before access can be given.
