Use this service to register to use GP online services.
You can use this service if you:
- are registered at the surgery
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- photographic identification and proof of address
Important: Please note
The patient will also need to visit the surgery with identification before we can complete your request.
If you are requesting access for a child under 11, you’ll need to bring the child’s birth certificate to confirm parental responsibility.
If the child is aged 11 to 16, they will need to complete a consent to share form at the surgery.
